Transform Housing is an innovative housing charity that provides support services to help vulnerable people change their lives and build better futures. We have been helping vulnerable people to build better futures for 38 years and we are supporting more people than ever before. Our client groups include: •Young people, including those leaving care •People recovering from alcohol and / or substance dependency •People with mental health issues •People with learning disabilities •People who have been in prison •Families in temporary accommodation •Older people •People with physical or sensory disabilities With 12 staff teams we are a major provider of supported housing for over 1,300 clients in Sutton, Surrey and in Wokingham in Berkshire. SCDT is a registered charity [no. 264133] and registered social landlord [no. H2452]
